Endovascular stent-graft for blunt thoracic aortic rupture: a novel and useful approach in selected cases.

چکیده
' 2003 Canadian Medical Association Blunt thoracic aortic rupture (BTAR) is a life-threatening injury that is responsible for 15% to 23% of deaths from motor vehicle crashes. Most victims die at the scene from exsanguination, but 15% to 20% reach the hospital alive because the rupture is contained by the aortic adventitia and the surrounding mediastinal structures. Up to 80% of patients with BTAR have concomitant injuries such as pulmonary contusion, neurologic trauma and intra-abdominal or pelvic hemorrhage. The accepted management of patients with BTAR comprises prompt diagnosis, evaluation and treatment of other lifethreatening injuries followed by open aortic repair. Early open aortic repair in the presence of associated injuries increases operative mortality and morbidity. Death after open surgical repair of BTAR ranges from 7% to 28% with a 4% to 36% risk of paraplegia. Recently, placement of an endovascular stent-graft has been proposed as an alternative to open surgical repair for BTAR, and excellent early results have been reported. We describe our experience with 3 cases of BTAR managed with endovascular stent-grafts and discuss the indications and the management issues related to this procedure.

Observations on the failure of stent-grafts in the aortic arch.
INTRODUCTION The results of endovascular stent-grafts in the abdominal aorta and descending thoracic aorta have been encouraging. Expanding the use of thoracic stent-grafts in to the aortic arch has been associated with increasing numbers of complications. Recently isolated cases of stent-graft collapse have been reported. METHODS This was a multi-centre European case series. Data was collect...

It still remains undetermined whether endovascular stent-graft placement (ESGP) is the optimal initial treatment for elective cases of thoracic aortic disease because of unknown long-term results. However, it is also recognized that ESGP contributes to better outcome as an initial treatment for aortic emergency, such as rupture, aortic injury, and complicated acute type B aortic dissection. Des...
متن کاملEndovascular stent - graft treatment for blunt aortic injury.
BACKGROUND Thoracic aortic injury resulting from blunt trauma is usually fatal and almost always associated with multiple, complex, nonaortic injuries that can adversely affect standard surgical repair of the aorta. Endovascular stent - graft treatment offers these patients a less invasive operative treatment option. METHODS AND RESULTS Between January 2002 and October 2003, 6 patients with b...
متن کاملEndovascular repair of traumatic thoracic aortic rupture: a single centre experience.
BACKGROUND Thoracic aortic rupture is usually the result of a sudden deceleration caused by a traffic accident, fall or some other misfortune. Before the endovascular era, there was only one treatment option: open repair, burdened by high morbidity and significant mortality. Now, we have the ability to treat it with a stent graft. The advantages of this method include avoiding a thoracotomy or ...
متن کاملAcute Traumatic Thoracic Aortic Injury: Considerations and Reflections on the Endovascular Aneurysm Repair.
Traumatic rupture of the thoracic aorta is a life-threatening lesion and it occurs in 10 to 30% of fatalities from blunt thoracic trauma and is the second most common cause of death after head injury. Immediate surgery is often characterized by a high mortality and morbidity rate. Delayed repair of traumatic aortic injuries has significant survival benefits and a much lower mortality rate compa...
